Our Pediatrician also said that the monitors give a false sense of security, and can also cause parents to have more anxiety over SIDS. In the bassinet, the bottom of it is a hard surface so no need for anything else. In her crib we have a large piece of very thin pressboard on top of the crib springs, then on top of that we put the monitor pad, then the mattress on top.

Some tips, if you're using the movement pad, make sure you have it sitting on a solid surface or it will definitely give you false alarms. I have the old angelcare movement Monitor we had when my first was a baby, set up for that, but we only use the movement pad with it and not the sound (because she is right next to me).

She sleeps in our room in a halo bassinest. However we have yet to put her to sleep in her crib. I have the video, sound and movement monitor set up in her bedroom with her crib. I have 2 actually (one old one that doesn't have video, and our new one that does have video) as I was also and still am terrified of SIDS. Also we used it with our first born, who is now 4.
